Primary Care Network (PCN) Staff

We have an extended team who are employed by our Primary Care Network (PCN) - https://www.burnleyeastpcn.com/ 

The PCN offers services and support to work collaboratively with the Surgery to provide services for our combined patient population of approximately 50,000. 

Burnley East Primary Care Network comprises of 6 practices:

• Prestige Medical Group
• Burnley Wood Medical Centre
• Yorkshire Street Medical Centre
• Thursby Surgery
• Colne Road Surgery
• Briercliffe Surgery

 Some of these services are delivered on-site, others maybe colleagues for which we refer into but of whom are associated to the care of our patients - these include:

• Managers
• Care co-ordinators
• Social Prescribers
• Pharmacists
• Physiotherapies
• Nursing Associates
• Community Matrins
• Care Home Nursing Team
• Mental Health Care Worker 

This team may provide care to you at our surgery, in your own home, at other practices within the PCN or at one of our community outreach clinics.
