Online Patient Triage

Due to new Government Requirements, from the 13th October 2025, all GP Practices must offer/use Online Patient Triage. Any request (medical or administrative) can be submitted via the online triage form available below or via the NHS App. Online triage requests will be actioned within 24 hours. If you cannot complete the online triage form, you can still ring the Surgery or come into reception as normal.

We have now gone live with our new online consultation service Online Patient Triage!

Online Patient Triage enables our patients to report non-urgent medical issues, submit an administrative request, and seek self-help guidance online. 

This is an easy way for you to contact our practice rather than waiting on hold to speak to us over the phone.

It is very simple to do - to start your request all you need to do is click the button submit a new request!

The main advantages of using the online paitent triage system are:

  • Ask about a non-urgent medical problem/routine appointment
  • Request a prescription
  • Request a sick note
  • Enquire about test results
  • Chase referrals
  •  Other related issues to save time instead of having to telephone the practice

Although the character count is limited on the form, please try to give as much specific and concise information as possible.

Any patient who does not have access to the internet or has difficulty completing the form will still be able to contact our reception team in the usual way.
