Collaborative Working With Other Healthcare Professionals

 

It is crucial that we work collaboratively with other healthcare professionals in our local area to be able to provide a better delivery of care for our patients. 

 

This enables our patients to be able to receive care that they would not normally be able to receive just from the Surgery alone.

 

We work closely with the following healthcare professionals:

  • Primary Care Network (PCN) East Lancashire Alliance (ELA) - Staff from our Surgery attend weekly meetings with the PCN along with all other East Lancashire GP Practices. We also attend any events that are available and share these with our patients if appropriate. The PCN ELA have various appointments that we are able to offer to our patients such as appointments with a First Contact Physiotherapist (FCP), Mental Health Triage, Remote or Face to Face appointments with a GP and/or Advanced Nurse Practitioner (ANP), Health Care Assistants and Practice Nurses. They also offer an Acute Visiting Service (AVS) for our housebound patients that need to see a clinician at their own home if they are not able to get to the Surgery.

  • Health Visitor - We work closely with the Health Visitors and have monthly meetings. We can also refer our patients to them.

  • Inspire - Our Lead GP holds regular reviews with the Inspire Team who support our substance misuse patients and we can refer our patients to them. 

  • Midwife - We refer our pregnant patients to the midwife who will support them in their pregnancy.

  • Counsellor - Our allocated TAPP clinician works with us weekly to offer appointments for our patients who would like to have counseling.
