Patient Partner
It is now possible to book, check or cancel an appointment via our automated telephone service 7 days a week, day or night.  All we need is your correct telephone number on our records.

Cancelling Appts
Please cancel your appointment if you no longer need it.  This can be done via Patient Partner or the Internet at any time - night or day.

Extended Surgery Hours  -   Appointments are available to see a Doctor between 7.00 and 8.00 a.m. on a Monday and 6.30 p.m. and 7.30 p.m. on Tuesday.

Advanced Booking
Appointments are available to be booked up to 4 weeks in advance.

Change of Address

Please let us know as soon as possible if your address or telephone number changes.

You may use the online form below to notify us of your change of details. The form will open in a new window. Fill out the details and press the Send button.

We offer a Text reminder service for appointments if we have a mobile number on our records.  Please let us have your mobile number if you would like to take advantage of this service.  (If you would like us to have your mobile number but not have text reminders then this is possible - please let us know)

Change of Address Form

If you don't wish to use the online form then drop into the practice and pick up a paper form.

If you have moved outside the practice area you will be informed appropriately and it will be necessary to seek registration at a practice nearer to your new address.
